Pay Telephone Service (PATS) Application
This page provides information about the Public Service Commission requirements
applicable to providing
Pay Telephone Service (PATS) within the State
of Florida.
To ensure the application you file with the Public Service Commission is complete,
you must access and carefully review all of the files listed below. If you
would like the Commission to mail you a copy of the application or if have any questions
regarding certification, please call the Certification Section at 850-413-6600.
Note: The application cannot be submitted electronically to
the Public Service Commission. The application form should be used for an
original certificate or for approval of sale, assignment or transfer of an existing
certificate.
